What You'll Do: In the role of MDS Coordinator / Nurse Assessment Coordinator, you will be responsible for completing and ensuring the accuracy of the Minimum Data Set (MDS) assessments for all residents. Your contributions will help shape personalized resident care plans and ensure proper clinical reimbursements for the services provided. Key Responsibilities: Evaluate Patient Driven Payment Method (PDPM) and related expenses during potential admissions. Assist in admitting new residents by assessing their nursing needs and determining appropriate reimbursement levels. Complete and verify the accuracy of the MDS process for all residents. Monitor Case Mix Index (CMI) scores, identifying any risks or changes that may influence reimbursement. Maintain high levels of revenue integrity while ensuring compliance with all state and federal MDS regulations and coding standards. Collaborate with interdisciplinary teams for accurate data collection during assessments. Provide insights and continuous education to facility staff and leaders. Experience in direct care, particularly in long-term care settings, or as an MDS Coordinator or Clinical Reimbursement Specialist is preferred. Familiarity with MDS 3.0, Electronic Medical Records (EMR), and Medicare/Medicaid requirements. A commitment to meeting the needs of elderly and chronically ill residents. Strong organizational skills, attention to detail, and the ability to make independent decisions. Excellent written, verbal, and interpersonal communication abilities.
